A patient randomly discovers a small renal cell tumour in ultrasound scanning. The Urologist talks to him about 4 therapeutic options: a) simple monitoring, b) Partial Nephrectomy, c) Cryoablation, d) Radiofrequency Ablation. Students cope with the problem and answer to questions on their computer, each one on their own. Once the procedure is over, their answers are processed so that the Moderators can base their comments presented thenext morning. 

A very common problem today: more and more patients who believe they have learnt everything about their medical problem and are convinced they know the indicative treatment, come to the doctor to actually 'demand' this treatment. And, despite the therapist physician's explanations, the patient insists! How do we, physicians, feel and how can we handle first ourselves and then our patient in such a situation. 

Thirty years since the first application of Extracorporeal Lithotripsy, nothing reminds of the past. Modern lithotripters, new focusing techniques, expansion of indications but also confusing dilemmas: which device? for which patient? when? how many times? And after lithotripsy, comes the big headache: elimination of stone fragments. How do we handle this? When should we intervene and how?

Many times PSA rises, but biopsies are negative. And then decisions are problematic. When should the second biopsy be conducted and using which technique? And what if it turns to be negative again? A significant problem both for the Urologist and the patient.

"Venous Leak" or elsewise "Venous-Occlusive Dysfunction" is a common pathology in patients with organic erectile dysfunction. However, we often see men 20-40 years old who had a history of excellent erectile capacity in the past to be diagnosed with "Venous Leak". How is it  that erectile dysfunction occurs in these young men with medical-free history? Could it be a medical fallacy or is it a real disorder?

Osteoporosis is not only a female privilege. Late-onset Hypogonadism and LHRH analogue therapy induce osteoporosis. The Urologist has to treat the cause, but is not aware of the primary disease. Also bone metastases compose a classical sign of metastatic cancer. An Expert comes to educate us upon the Bone Disease.

Within 1 hour, the Coordinators "run" the case and comment on the recorded mistakes made by the Trainees. They all together provide final final answers.

Everyday clinical practice includes clinical examination, which is highly subjective as an assessment. In many cases this evaluation happens to be so personal that even when recorded in the Urologist's personal notes it does not help at all in the next clinical examination. There have been lots of attempts aiming at objective evaluation systems but not many of them are widely accepted.
